Filing claims doesn’t have to be time-consuming or difficult! Use our online claims filing through your secure Login.

Why e-File?

  • Priority processing: these claims are reviewed within 2 business hours of receipt of your faxed claim. Avoid mail delays & get priority review of your items!
  • Eliminates hard to read claims (due to using light pens or pencils or interpreting handwritten details).
  • Eliminates mail delays caused by US mail.
  • Immediate acknowlegement that your claim is received. Complete the claim form & see it pending on your account after you “submit” it in Step #6 below. View your account 24/7 and see that your documentation was received & your claim has been reviewed, only 2 business hours later!

Here's how it works:

  1. Go to our secure Login
  2. Login to your account
  3. Click the eFile link
  4. Choose the plan year & account from the drop down lists
  5. Key in the claims detail: date of service, type of service, etc.
  6. Click the “Submit” button
  7. If you have another item to submit, do #3-6 above for each of the next items/expenses you are claiming. If you only have one expense to submit/claim or you are finished listing/submitting items, go to #8.
  8. Print your completed claim form (fax cover sheet) by selecting please print a Fax Cover Sheet
  9. Sign the claim form! This is an important step.
  10. Fax the signed claim form and your supporting documentation to the number on the claim form. (Use the fax number on this form to get into our priority grouping.)
